Receiving dental services

Most dental providers will submit claims to insurance companies as a service to their patients. If, however, your provider does not provide this service, you must submit a claim to us. If you have questions about filing a claim, call our Dental Department at (800) 279-4000.

Please keep in mind that your claim must include the following information:
  • The name, address, and Social Security number of the covered employee.
  • The employer’s group number. This is listed on your insurance identification card.
  • The patient’s name and date of birth.
  • Information regarding any other group insurance coverage.
  • The dental provider’s name, complete address, and federal identification number.
  • The patient’s diagnosis and the appropriate procedure code for each service received by the patient with an itemization of charges for each service.
Any dental claims submitted to us must use accurate, valid American Dental Association (ADA) codes from the most recent edition of Current Dental Terminology (CDT) manuals. For more information about claim procedures, please refer to Section 9 in your group dental policy.
