Fall 2008 Health Assessment


Note: The Health Assessment will be available until October 31 at midnight. After that point you will still be able to login and review your results, but you will not be able to enter or change any information.

Who can take the Health Assessment?

WEA Trust health plan subscribers and spouses for whom the Trust is their primary insurer.

If you have the Trust as your secondary health insurer you're not eligible. This includes spouses covered under their employer's health plan, and retirees and spouses with Medicare as their primary insurance. In addition, individuals who have COBRA continuation health plan coverage are not eligible.

For more comprehensive Health Assessment results, please have the following information ready to enter into the assessment:

  • Subscriber number, listed on your insurance ID card (required)
  • Blood pressure (optional)
  • Cholesterol level (optional)
  • Blood sugar (glucose) readings (optional)
  • Waist circumference (optional)
    How to Measure Your Waist Circumference:
    Place the tape measure around the smallest area above your belly button. Make sure the tape measure is level all the way around and does not push in or indent the skin. Relax, exhale, and measure.

If you don't have your blood pressure, cholesterol, blood sugar numbers, or your waist circumference, you can get them by calling your doctor's office. If you haven't taken these tests recently, you should talk with your doctor about scheduling them.

Who sees the results of my assessment?
You can feel confident about the security of your Health Assessment results. Your personal health information is protected under federal law, and your employer does not have access to your Health Assessment results. Furthermore, claims and eligibility decisions are not based on your Health Assessment results.

Who does see the information? WEA Trust health professionals use the results to offer you programs tailored for your specific needs. Also, based on Health Assessment results, some
WEA Trust members are offered the opportunity to work with a personal health advisor from Healthways.
